Switching pharmacy on the Wegovy pill: what to send, when to move
Moving to a cheaper pharmacy mid treatment is normal and usually simple. You do not need a letter from your old pharmacy. You need two pieces of evidence, a few days of tablets in hand, and a bit of timing.
In one paragraph: you can switch pharmacy between any two orders unless you are inside a minimum term. Photograph the label on your current box and screenshot your last order confirmation. That is the evidence a new pharmacy wants. Start the new consultation while you still have several days of tablets, because the check takes a few days and most pharmacy operations close over the weekend. Most pharmacies will ask whether you want to stay at your current strength or step up, so the choice is generally yours. Cancel the old plan only once the new one has dispatched. We do not advise long term commitments, so check what you are signing before you take a 3, 6 or 12 month plan.
- You do not start again at 1.5mg. With proof of your current strength and no missed tablets, you ask the new pharmacy for the same strength or the next one up.
- Days at your old pharmacy count. The licence sets a minimum of one month at each strength. The month does not reset when you change supplier.
- The new prescriber decides. You make the request. The prescriber checks your record and writes the prescription.
Your dose when you swap pharmacy: start again, stay or move up?
You do not start again. A new pharmacy treats you as someone already on treatment, as long as you show what you have been taking. With proof of your current strength and no missed tablets, you ask for one of two things: the same strength, or the next strength up. The new prescriber checks the record and writes the prescription.
- Stay on the same strength. Always open to you. The Wegovy tablets SmPC lets a prescriber hold you at a lower strength for longer when you need more time.
- Move up one strength. The SmPC sets “a minimum duration of 1 month at each dose level”. Days on your current strength at the old pharmacy count toward the month.
- No skipped strengths. The order is 1.5mg, 4mg, 9mg, then 25mg. A new pharmacy will not take you from 4mg to 25mg. One tablet a day is the limit. The SmPC says taking more than one tablet a day “should not be done to achieve the effect of a higher dose”.
- Move down. Ask for a lower strength if side effects are hard to live with. The prescriber decides.
The next strength open to you from each dose
| Strength you take now | Stay on | Next strength up | Earliest move up |
|---|---|---|---|
| 1.5mg once daily | 1.5mg | 4mg | After a minimum of 1 month on 1.5mg |
| 4mg once daily | 4mg | 9mg | After a minimum of 1 month on 4mg |
| 9mg once daily | 9mg | 25mg | After a minimum of 1 month on 9mg |
| 25mg once daily | 25mg | None. 25mg is the maintenance dose and the highest licensed daily dose | Does not apply |

How a month is counted on a daily tablet
The UK SmPC says one month. The United States prescribing information for Wegovy tablets lays out the same schedule in 30 day blocks: days 1 to 30 on 1.5mg, days 31 to 60 on 4mg, days 61 to 90 on 9mg, then 25mg from day 91. On a daily tablet, a month means around 30 mornings with a tablet taken. Count the tablets you swallowed, not the days since the parcel arrived.
Your proof is the dispensing label on the tablet pack. The label shows the pharmacy, the strength and the date of supply. A label dated 30 or more days ago, with an honest count of any missed mornings, tells the new prescriber where you stand. Photograph the label before you throw the box away.
Missed tablets change the answer
The rule for one missed tablet is short. The SmPC says: “If a dose is missed, the missed dose should be skipped, and the next dose should be taken the following day.” You take one tablet the next morning and never two.
Daily dosing makes a switch less forgiving than a weekly injection. A delivery gap of four days means four missed doses in a row. The UK SmPC, the UK patient leaflet and the United States prescribing information give no rule for several missed days in a row, and none of the three sets a point where you must restart at 1.5mg. We found no official number of days. The prescriber decides, and a lower strength becomes more likely the longer the gap.
| Your situation | Dose to ask for | Basis |
|---|---|---|
| No missed tablets, less than 1 month on your strength | The same strength | SmPC section 4.2: minimum of 1 month at each dose level |
| No missed tablets, 1 month or more on your strength | The same strength or the next one up | SmPC section 4.2 |
| One tablet missed | The same strength. Skip the missed tablet and take the next one the following day | SmPC section 4.2 and the patient leaflet, section 3 |
| A few days missed, for example a late parcel | The same strength. A move up will often wait until you have a full month of tablets taken | No rule in the SmPC or leaflet. Prescriber decision and pharmacy policy |
| Weeks without tablets | Whatever the prescriber judges safe. A lower strength, down to 1.5mg, is more likely the longer the gap | No restart rule in the SmPC, the leaflet or the United States prescribing information. Prescriber decision |
| Side effects on your current strength | The same strength for longer, or a lower one | SmPC section 4.2 allows holding at the lower dose level. United States prescribing information: “consider delaying dosage escalation” |
| Moving from the Wegovy 2.4mg weekly injection | 25mg tablets, starting one week after your last injection | SmPC section 4.2 |
Our Wegovy pill missed dose guide covers the skip rule in more detail.
What to tell the new prescriber
- The strength you take now: 1.5mg, 4mg, 9mg or 25mg.
- The date you took your last tablet.
- How long you have been on this strength, with the date on the dispensing label as proof.
- Every missed morning, with true dates. A prescriber who finds a gap you left out has reason to doubt the rest.
- Side effects on this strength, and how you are coping with them.
- The dose you want: the same strength or the next one up.
The way you take the tablet stays the same at every pharmacy: one tablet on an empty stomach after at least 8 hours without food, a sip of water up to 120 mL, then a wait of at least 30 minutes before food, drink or other oral medicines. People settled on 25mg will find the long term picture in our Wegovy pill maintenance guide.
Why people switch, and why it is worth it on a daily tablet
The Wegovy pill is priced per strength, and the pharmacy that was cheapest at 1.5mg is rarely the cheapest at 25mg. Because you spend far longer on maintenance than on any starter dose, a switch at the right moment can save more over a year than any welcome code. It is also how you escape a pharmacy whose service has slipped, whose stock keeps running short, or whose subscription has crept up in price.
The daily tablet adds one complication that a weekly pen does not. If you run out, you lose several mornings in a row, not one Thursday. That is why timing gets its own section below.
The evidence a new pharmacy will ask for
A photo of your current box label
Clear, in focus, showing the strength, the dispensing pharmacy and the date. This proves what you are on and that it was supplied properly. Take it before you are down to the last strip.
A screenshot of your last order
The confirmation email or your account page showing the pharmacy, the strength and the order date. Together with the label it gives the prescriber an unbroken history without any paperwork from your old pharmacy.
Timing: start with a dose in hand
A new pharmacy has to run its own consultation before a prescriber can approve you, and that takes a few days at the best of times. Most operations do not dispatch at weekends, and some do not review at weekends either. Start the switch when you have at least five or six tablets left, not on the morning you open the last strip. If the review is quick you simply have a small overlap, which is far better than a gap.
The switch in five steps
Choose on month two price, then check month one
Shortlist pharmacies on the retail price list at your strength, then see who has the best first order offer on the discount price list. Check the pharmacy on the Pharmacy Score page before you decide.
Gather the two pieces of evidence
Box label photo and last order screenshot. Keep them in one folder on your phone so you can upload both in the consultation.
Start the new consultation with tablets to spare
Answer the questionnaire honestly, declare your current strength and how long you have been on it, and upload the evidence when asked. If you are due to step up, say so. The prescriber decides, but only on what you tell them.
Wait for dispatch before you cancel anything
Only once the new pharmacy confirms dispatch should you cancel an old subscription, and only within its notice period. If you were paying as you go, there is nothing to cancel.
Keep the routine the same
Same tablet, same strength, same empty stomach routine. A new pharmacy changes who you buy from, not how you take it. Our guide to taking Wegovy pills covers the routine if you want a refresher.
When you cannot switch straight away
If you signed up to a multi month plan, check for a minimum term or a clause recovering your welcome discount when you leave early. You can still switch, but you may need to serve the notice period or accept the clawback, so weigh the cost against the saving. Our subscription versus pay as you go guide explains what to look for in the terms.
We do not advise long term commitments. A 3, 6 or 12 month plan ties you to one pharmacy at one price. Our guide to Wegovy pill subscription plans sets out the drawbacks.
ID checks at the new pharmacy
A new pharmacy will usually verify your identity as part of the consultation. If you do not hold a passport or driving licence, a PASS card is accepted by many providers. Our guide to ordering without a passport covers what to send.
Questions people ask
Do I have to start again at 1.5mg when I switch pharmacy on the Wegovy pill?
No. With proof of your current strength and no missed tablets, you ask the new pharmacy for the same strength or the next one up. The new prescriber checks your record and makes the decision. A long break in treatment is the main reason a prescriber restarts someone on a lower strength.
Can I move up a dose at the same time as I switch pharmacy?
Yes, if you have taken your current strength for at least one month. The Wegovy tablets SmPC sets a minimum of 1 month at each dose level, and days at your old pharmacy count toward the month. The order is 1.5mg, 4mg, 9mg, then 25mg, with no skipped strengths. The dated dispensing label on your tablet pack is the proof.
My tablets ran out for a few days during the switch. What dose will I get?
On a daily tablet, each day without a tablet is one missed dose. The SmPC covers a single missed dose: skip the missed tablet and take the next one the following day. No official rule covers several missed days in a row, so the prescriber decides. Give the true dates and ask for the same strength. A lower strength is more likely the longer the gap.
Does the empty stomach routine change with a new pharmacy?
No. The tablet is the same medicine from every UK pharmacy. Take one tablet a day on an empty stomach after at least 8 hours without food, with a sip of water up to 120 mL, then wait at least 30 minutes before eating, drinking or taking other oral medicines. Never take two tablets to make a higher dose.
Do I need a treatment summary or letter to switch pharmacy?
Not usually. A clear photo of the label on your current box and a screenshot of your last order confirmation are the evidence most UK online pharmacies ask for. Some may request a short treatment history in the consultation form.
Can I switch pharmacy at any time?
Yes, unless you signed up to a multi month plan with a minimum term or a promotional clawback. Outside of that, you can move between any two orders.
Should I take a 3 month or 6 month plan?
We do not advise long term commitments. A 3, 6 or 12 month plan ties you to one pharmacy at one price, and leaving early usually costs you. Our guide to Wegovy pill subscription plans sets out the drawbacks.
Will I be pushed up a strength when I switch?
No. Most pharmacies ask whether you want to step up, so the choice is generally yours. Ask to hold at your current strength and most prescribers will agree, as long as holding is clinically sensible.
Will the new pharmacy let me carry on at my current dose?
Most will continue you at your current strength or move you up a step if you are due, provided your evidence shows what you have been taking. The decision is the prescriber’s and they may ask further questions.
What if I have tablets stockpiled from the old pharmacy?
Upload the order screenshots and box label photos for those supplies together, so the new prescriber can see an unbroken history. Gaps or unexplained extra boxes are what slow a switch down.
